Managing hospital resources is a key strategic challenge in high-volume tertiary care, where turn-around time (TAT)—the time interval from the physician discharge order to physical. In this mixed-methods study of 320 inpatient cases in an Indian tertiary hospital, we utilize quantitative HIS time-stamp data and qualitative stakeholder perspectives to identify systemic bottlenecks. The results show that most discharges are timely, but a substantial proportion fail to meet benchmarks because of the administrative complexities brought by insurance-linked cases (TPA and PSU approvals), delayed billing processing, and logistical challenges. There was a significant temporal mismatch in late exit patients that resulted in them leaving at times when admissions are starting again in the early morning, which creates ED congestion and does not allow the most appropriate use of the beds (e.g., for patients being observed post-discharge). To tackle these challenges, the work suggests a formal structure that includes real-time digital dashboards, dedicated discharges coordination, along with the creation of lounges for discharge to liberate clinical recovery from bed occupancy. These evidence-based solutions are presenting a scale model roadmap for the healthcare executives to advance the hospital through put, financially sustainability and the patient care experience in healthcare facilities.
